Good question. I can't find an example of this being awarded in MLB of late.
The actual rule is a 3-base award for a fair batted ball contacted with 'detached player equipment or illegal glove'. So much more than just a hat. If a fielder throws their glove at a ball as it goes past and it touches it or if they catch it with a glove that is then deemed illegal it would be 3 bases as well.
It must have been an issue at some point or they wouldn't have bothered with a rule. Becoming an umpire was when I learned of it, and its still an oddity in the rules. Why 3 when all the others are 1 or 2? If I was king, this is one of the rules I'd make consistent with the other base awards as a way of simplifying an already complex rule book.
